We provide Community Residential Rehabilitation services in an Unlicensed Provider's owned, rented or lease apartment settings. Our residences are spacious and bright, the environment is intended to simulate a typical household, with all the comfort of home.
We provide service to individuals with developmental disabilities and autism.
We support our residents with continual assistance to complete daily living and self-care tasks.

PHILOSOPHY
Being an intellectually disabled person does not infringe upon Consumer human rights, which Trinity Human Services valued at large for all our Consumers.
The basic process of promoting growth involves doing for the person, doing with the person, and having the person do for themselves; therefore, helps to avoid dependency

Services Provided
Unlicensed Residential Habilitation
This Residential Habilitation is being provided to Participants or Consumers who lives in unlicensed Providers'-owned, rented, or leased apartment settings, as stated in 55Pa. Code 6400.3(f) (7) licensing regulations exclude Community Homes that serve 3 or fewer Individuals with intellectual disabilities / autism with age ranging from 18 years or older, who needs a yearly average of 30 hours or less of direct staff contact per week per home.
As an Unlicensed small group home of one, two, or three individuals, we provide room, board and services to very vulnerable individuals, such as individuals with developmental disabilities/Autism.
Our services are individualized and driven by the needs and goals of the individual and his or her loved ones, with a focus on increasing independence.

Respite Care - Out Of Home
We provide Respite Care- Out Of Home in the Provider's-owned, rented, or leased apartment settings to individual with developmental intellectual disabilities/ autism.
This service is provided to supervise and support individuals on a short-term basis due to the absence or need for relief of those persons normally providing care.
Whether for planned or emergency situations, giving the person(s) normally providing care a period of relief that may be scheduled or due to an emergency.

Our Commitment
We are dedicated to helping our Consumers who need less then 30 hours of weekly supervisions.
We assist individuals in acquiring, retaining, and improving self-help, socialization, and adaptive skills.
We want families to feel part of their family member’s life and experiences at Trinity Human Support Services. Families play an important role in orienting new staff to their family member’s background and other important information.
Every effort is made to assist individuals in establishing and maintaining natural support networks involving family members and close friends.
